Lack of sharpness in mind-major source of accidents at sea

After less than an hour under way and in total darkness I was at the end of launching the paravanes when I saw loads of moorings passing on our port side.
It only took me a second to realize we were heading straight into a tunafarm and I screamed to Mercedes to go into reverse to stop Dora Mac.
With the handheld searchlight we got a reasonable a picture over our situation and by going slowly on reverse from where we came, we managed to get out of the farming system without any injuries to farm or ship.
There were no tuna farms or lights on the Navionics chart on the Raymarine chartplotter ,but the reason for our failure was obvious. We were navigating on charts from 2016 and switching over to our updated Navionics on the mobile phones the farm and it´s warninglights were well marked.
I made our route setting all waypoints but with lack of sharpness in my mind I failed to look for important updates. That´s also my experience from long time cruising that catastrophies at sea often emanate from a dull mind and not from lack of knowledge.
